Criterion told me to contact Disney, who sent me a new insert directly.
There's a "contact us" here
So I did contact Disney and this is the response I got. I also mentioned that I have the Criterion version:
Chasing Amy is no longer a Disney affiliated release and now belongs to Lionsgate. Please contact Lionsgate at (310) 314-2000 or online at http://www.lionsgate.com" onclick="window.open(this.href);return false;.

Lionsgate reissued Chasing Amy last summer.

I wonder if they used the Criterion banner without asking permission? The Lionsgate release certainly isn't out of print. One of the dangers of doing work for hire and letting other companies own your work.

The Lionsgate reissue could have been them releasing unsold Disney stock. I think, when Lionsgate first took over the catalog, their reissues of already available Blu-rays were just unsold Disney stock with a new bar code stuck over the old bar code.

At my local Half Price Books store, dvds of Chasing Amy have long been priced at 4.99 (or less even) until this week when, for first time, in-store copies of Chasing Amy suddenly have an "Out of Print" sticker on their covers and price is higher too. I asked a friend who works part time there about the changes, apparently their pricing for OOP Criterion dvds is taken from Amazon dealers' changes and not a set HPB store policy.

Are we sure about that? Recently, when my gift of Armageddon (also Buena Vista) arrived without a booklet, Criterion nicely sent me one when I inquired.

All I know is that Buena Vista does the pressing of the releases, including the booklets. I don't know about what kind of extras Criterion has lying around.

Well, I'll just count that as definitive from the source.
Probably just a case-by-case basis for the booklets. Cafeman, you should email Mulvaney ([email protected] )to find out.

I noticed this when I got my Blu of No Country For Old Men, since I thought I was getting the Lionsgate reissue, but it was definitely a Disney release.

My copy is also missing an insert. I reside in Australia, so don't like my chances of getting one sent out.

Is anyone able to scan theirs in high def & email it to me, so I can print out my own copy.
