That's never been the rule, as far as I can tell looking back to early film posters. They often just follow the order of billing.

I'm sure there are contractual obligations concerning the placement of names, and they won't always coincide with the positions of the actors in the photo.

I think that's clearly the case, but that just begs the question: why not just reposition the actors? Actors' contracts are signed way before the publicity stills are shot (that is, when the art isn't photoshopped to begin with).
