After reading about this here, I couldn't believe I did not notice such an error after having viewed the Arrow Blu-ray twice since it was released. Checking it now, I can see why viewers missed it. As far as these kind of mistakes go, this one is relatively seamless. The repeated shots are primarily three reaction shots from the scientists interrogating Cole (Willis). Since Cole is engaged in a monologue throughout this section, it's not illogical that the film cuts to show the scientists listening and reacting to him (and audio synchronization is not affected since we're not seeing Cole's face during the section). For me, this is a classic example of the Kuleshov effect: while the three repeated reaction shots are identical, it seems like the scientists' facial reactions are different due to the change in context of what Cole is telling them. This error could even be mistaken for directorial intent with Gilliam commenting upon the implacability of the authorities by reusing the same reaction shots!
Still, for the record, it's disappointing this error got through and I wish it had been caught earlier and Arrow would have done an exchange.
I felt the bigger clue as something possibly being wrong is when Cole is shown the 12 Monkeys logo at a moment that doesn't seem right within the discussion and with nobody bouncing on this, only to be shown it a 2nd time and only then the discussion following thtough this. But otherwise yes, since the sound isn't looped, the scientists reactions feel edited at the right places despite the mistake.

I agree. While I've not reviewed any other copy of the film to confirm the proper editing, the first time we see the reaction shots of the scientists (and the projection of the 12 Monkeys logo) is the error and the second time those same shots appear is the correct placement, right?
"We didn't notice it but neither did the filmmaker". That may be true but between them claiming that they (rather than having had to point it out to them by customers!) spotted the errors on the Henry UHD and BDs and feeling compelled to point out that Terry Gilliam did not spot this either, I feel like Arrow could have handled both error acknowledgements with a bit more grace and humility. I think most people would be fine with them slowing down a little on the UHD releases if it meant fewer fuckups in QC (yes, the error on Henry and 12 Monkeys was baked into the source provided to Arrow but the BD transfer was definitely on them and ought to have been caught before the discs went to the manufacturers).
I think this is just a statement release, phrased as a usual statement release. It certainly could have phrased in a more accurate way to reflect more what happened, but this seems like just a relatively boilerplate statement.
According to some posts on the Blu-ray.com forum, Arrow will be fixing the Blu-ray on future pressings, but will not be offering replacements. Arrow continues having absolute shit customer service.
I gotta hand it to them for the spectacular dodging of the Blu-ray in that FAQ. way to not even acknowledge it's a thing at all!
hopefully James F or someone reasonable at Arrow can see the ridiculousness of this. it's not like this is a 10 year old DVD we're asking them to replace, Bluray still has to be the majority of their market

Not a problem. I don't take forum stuff personally. This isn't real life and there is enough awful stuff to stress out about in real life. Sorry if my response sounded like I did take it personally.
